In male SD rats given a single oral dose of ondansetron (10 mg/kg) with Ijintang-gamibang (IJTGMB, 100 mg/kg) co-administered orally within 5 min, the plasma concentration profile of ondansetron did not differ from the control, and none of the pharmacokinetic parameters (Tmax, Cmax, AUC, t1/2, MRTinf) differed between the two treatments. The same was true for the initial dose of the repeated-dosing study after 6 days of IJTGMB pretreatment [​2​].

[Expert interpretation]
Ijintang-gamibang does not affect the pharmacokinetics of Ondansetron on single co-administration, as it alters neither the absorption nor the elimination of Ondansetron, with or without 6-day pretreatment.

[Interaction]
After 8 days of repeated co-administration (following 6 days of IJTGMB pretreatment), plasma ondansetron was detected at lower concentrations and for a longer duration in the combination group than in the control. AUC0-t decreased from 460.82 +- 150.84 to 281.15 +- 60.21 ng*h/mL and AUC0-inf from 473.32 +- 164.17 to 291.54 +- 59.10 ng*h/mL (both p<0.05); Cmax fell from 327.60 +- 57.35 to 237.60 +- 72.96 ng/mL, while t1/2 (0.66 -> 0.78 h) and MRTinf (1.35 -> 1.33 h) were largely unchanged [​2​].

[Expert interpretation]
Repeated co-administration of Ijintang-gamibang significantly reduces the bioavailability of Ondansetron by interfering with its absorption while leaving t1/2 and MRTinf unchanged; the authors suggest separating the two by an interval longer than the MRTinf of Ondansetron.

[Interaction]
In a rat model of oral ulcerative mucositis (OUM) induced by 5-fluorouracil (40 mg/kg i.p., three times at 2-day intervals) followed by 50% acetic acid application, oral administration of 1% hangeshashinto (HST) in the diet significantly lowered the OUM score on days 6 and 7 after acetic acid treatment and significantly increased body weight on day 6 (274.3+-7.7 g vs 260.7+-6.7 g). Pharmacokinetic analysis showed that plasma concentrations of the active ingredients after oral HST were below the effective concentrations for keratinocyte migration, suggesting a direct local action of HST on the oral mucosa rather than a systemic one [​1​].

[Expert interpretation]
Banhasasimtang accelerates the healing of 5-FU-induced oral ulcerative mucositis through direct local action at the ulcer site rather than systemic exposure, as plasma levels of its active ingredients remain below the concentrations effective for keratinocyte migration.
