In HeLa cervical cancer cells, pre-treatment with a low dose of Hochu-ekki-to (HET, 50 ug/mL for 24 h) - which alone did not affect cell viability - enhanced cisplatin-induced apoptosis. Flow cytometry showed a distinct increase in the sub-G1 population. Mechanistically, HET down-regulated p-Akt and the Bcl-2/Bax ratio, and stimulated cisplatin-induced up-regulation of p53 and active caspase-3 [​3​].

[Expert interpretation]
Bojungikgi-tang enhances Cisplatin-induced apoptosis through down-regulation of p-Akt and the Bcl-2/Bax ratio and up-regulation of p53/caspase-3, thereby increasing the Cisplatin sensitivity of cervical cancer cells.

[Interaction]
In cisplatin-resistant lung cancer A549/DDP cells, Bu-Zhong-Yi-Qi decoction (BZYQD) showed direct cytotoxic and chemosensitizing effects. Cotreatment with BZYQD and cisplatin induced the intrinsic apoptotic pathway and also activated autophagy, as indicated by increased LC3 puncta and accumulation of LC3-II and ATG7 protein [​4​].

[Expert interpretation]
Bojungikgi-tang potentiates Cisplatin cytotoxicity through induction of intrinsic apoptosis and autophagy, thereby contributing to the reversal of Cisplatin resistance.

In a phase I randomized, open-label, crossover study in 10 healthy male subjects and a phase III randomized, placebo-controlled, double-blind, parallel study in 43 ischemic stroke patients, co-administration of Bojungikki-tang (BJIKT) with aspirin did not alter the pharmacokinetics of acetylsalicylic acid (ASA) or salicylic acid (SA): mean AUC, Cmax and Tmax did not differ between the BJIKT and placebo groups in either population (e.g. ASA AUC 60,959.5+-14,243.3 vs 53,465.3+-6,777.6 ng/mL*min; Cmax 440.8+-100.8 vs 418.2+-81.5 ng/mL). Pharmacodynamically, plasma thromboxane B2 concentrations and platelet aggregation were also unaffected by BJIKT co-administration [​2​].

[Expert interpretation]
Bojungikgi-tang does not cause a clinically relevant herb-drug interaction, as it affects neither the pharmacokinetics of acetylsalicylic acid/salicylic acid nor the antiplatelet pharmacodynamics of Aspirin.
